Discharge summary processing

Convert discharge summaries and related hospital records into structured, searchable data with clinician or coder review where needed.

Documents in

  • Discharge summaries
  • OPD consultation notes
  • Outpatient prescriptions
  • Lab reports
  • Radiology reports
  • Operative reports
  • Referral letters

How it runs

  1. Intake

Records are uploaded or sent via API, individually or as combined patient files.

  2. Text extraction and OCR

Text is extracted from digital files and scanned pages are read with OCR.

  3. Classification and schema matching

Each record is classified and matched to a schema from the India healthcare pack, with page ranges identified inside combined files.

  4. Field extraction

Diagnosis, procedures, medications, dates and follow-up instructions are extracted with confidence values.

  5. Human review

Fields below the confidence threshold are held for a reviewer, who corrects and approves.

  6. Summary and search

A summary is generated, and records can be searched and queried through chat.

What you get

  • Structured discharge record with per-field confidence
  • Combined patient files separated into individual record types
  • Reviewer-confirmed clinical and administrative fields
  • Searchable record set with summaries

Where review fits

Clinical documents are often handwritten or inconsistent. Review lets a person confirm uncertain fields before downstream use. Corrections are recorded and used as context for later extractions with the same schema.

Integration points

Structured results are available via API and webhooks. Sensitive health data is handled under access controls and audit logging that support a customer’s own privacy obligations. Single-tenant deployment in the customer’s Azure is available for teams that need it.
